We are looking to hire a Staff Nurse to join the team. The staff nurse is a registered nurse who is responsible for ensuring competent, compassionate nursing care for specific patients and families. The staff nurse is responsible and accountable for the overall nursing care management of patients within the outpatient Infectious Disease Unit including primary care for People Living With HIV/AIDS and Infectious Disease specialty care. The staff nurse also collaborates with other health care providers, attending physicians and community-based organizations in obtaining optimal health outcomes for patients. The Staff Nurse meets patients’ needs efficiently and expeditiously by continuously improving the patient’s experience, helping to ensure the institutional standards of high-quality patient care, reducing costs and ensuring reimbursement. Through broad knowledge of clinical care and systems management, the staff nurse evaluates, predicts, and facilitates the trajectory of patient care. PRINCIPAL DUTIES AND RESPONSIBILITIES: CLINICAL - Triage and assess patient needs via telephone or in basket. - Provide initial patient assessment and evaluation for walk-in patients. - Perform phlebotomy, provide SQ/IM injections and occasionally, place peripheral intravenous lines for single dose IV infusions. - Perform PICC line dressing changes, PICC line removal, and line malfunction assessments including administration of Alteplase. - Provide care and education to patients who are on HIV antiretrovirals, PrEP/PEP, and antimicrobialtherapies. - Provide testing and education about treatment and prevention of STI’s (sexually transmitted infections). - Perform Prior Authorizations for Specialty Medications, coordinate medication refills and monitor medication adherence. CASE MANAGEMENT - Collaborate closely with the NP, Fellow and/or Attending MD to coordinate and implement the comprehensive plan of care. - Work with the Community Health Worker team to coordinate outreach and follow up for patients living with HIV and to re-engage patients who have fallen out of care. - Monitor patients at home or in SNF on IV antimicrobial therapies with side effect monitoring, weekly laboratory monitoring, weekly line care and - Identifies key barriers to care and patient’s ability to manage their health and wellness through initial and on-going assessments. - Assist in referrals to Community Based Organizations for resources and services. - Assist in the coordination of home-based services such as VNA or Food Delivery. - Monitors the patient’s progress, intervening as necessary and appropriate to ensure that the plan of care and services provided are patient focused and friendly, high quality, efficient, and cost effective. - Facilitates access to the health care delivery system along the continuum, by navigating any barriers to care and advocating on the patient’s behalf. - Works closely with the Division’s Benefits Manager and Patient Financial Services (PFS) to assist and support patients with insurance coverage and to facilitate enrollment in HIV drug assistance programs as appropriate. - Collaborate with the multidisciplinary team to facilitate Emergency Department avoidance pathways to care including outpatient initiation of IV antibiotics, urgent care or specialty care coordination. QUALITY/ADMINISTRATIVE - Identifies and helps to resolve service delivery problems and recommends methods to improve and streamline services. - Participate in Division quality assurance activities. - Participate in social service rounds and other interdisciplinary meetings. - Performs other duties as assigned.
